Patient Eligibility 

In order to be seen at HCHC, you must 

• Be 18 years of age or older

• Be uninsured

• Be considered financially eligible based on Total Income and Family Size

**We do not see Veterans as they are required to go to the VA. 

Services we Provide

• Primary care services, such as diabetes management, annual physicals, colds/flus, etc.

• Medication refills, basic blood work orders
